No its not on the glass the bottom of the light fixture is about 2-3" above the glass top then the actual light is another 2" from that. I can tell you the glass is hot just cool enough to touch but not for too long. It also happened on my planted tank upstairs and that fixture is sitting on the glass but the other glass is just fine no problems.

It might be the heat. EK had this same problem with one of his glass tops.

But it was odd because only that one glass panel would crack. He had to replace it like 3 times.

Is the MH fixture sitting on the glass top?

You can rig the fixture up to the canopy some how to have it not sitting on the glass.

I put my new lights on the tank and fired up a couple of the MH. Under one the glass top I have cracked in half and a big piece fell into the tank. Is this caused from the heat? If so what can I do add a fan?

I just picked up a bad @$$ light set-up from Dustin. Comes with 3 400w MH and 2 T12. Thats around 1.5 kw
