What all have you tried feeding him? We have 2 fahaka's, one is about 6" and one is about 12".
The larger one LOVES clams on the 1/2 shell, ghost shrimp, raw shrimp from the grocery store, and krill. He'll eat mussels if that's all I offer, but not his favorite.
The smaller one won't touch any of that stuff (except ghost shrimp) and only wants bloodworms and snails. Our Fishhouse
Sleep: A completely inadequate substitute for caffeine.

Have you tried letting him be in a tank alone? Our larger puffer gets freaked if other fish are with him. We put some feeders in with him once and he didn't like it. He just curled up in the corner and sulked until I pulled them out. Our smaller one kills anything we put in there with him. My point is that puffers have such distinct personalities and it's really important to figure out what yours likes and dislikes.
Going back to the cloudy eye issue...I know there was an assumption that he burned his eyes on a heater, but cloudy eyes in a puffer could be indicative of parasites or internal bacterial infection. You said his eyes look better, so he could be on the mend.
Here are 2 threads to read. edmlfc went through some horrible things with her fahaka and internal parasites. She's good about posting pics, so you can see if your puffer has any of these symptoms.

and

I'll see what else I can dig up that edmlfc has documented. She went through hell with this puffer. Our Fishhouse
