    I just heard there was an explosion at one of the plants in Pasadena. All our Southeast 'boxers okay?

        Seems like all is well.

        I know they issued a shelter in place for the immediate surrounding area.

        Our teams have not been activated yet which usually happens during shelter in place orders so it must not be to bad.

                I am alive.

                A friend of mine used to work in that plant... he is very glad to not be working there anymore. He told me about some things in that plant, does not suprise me.

                  The article says it has been cited over 20 times since 2004. I'm not sure what the "standard" is for citations on industrial chemical processing, but 0 seems like a good number for me. I like how the article says the citations were for minor things like monitoring valves... except that valves are what keep pressures and concentrations in check, and prevent things like, explosions!

                  People will probably get a slap on the wrist and they will continue with what they were doing. Maybe a fine that equates to .0001% of someone's paycheck. Sad day

                    One of the morning shows the other day said that the plant had never since it had been built in 2002 in its current format ever been inspected.. Simply to large of a backlog.

                      Your first external inspection has to occur in 5 years after being put into service.

                      Depending on your corrosion rate, your first internal inspection occurs 1/2 of your remaining life or 10 years for a pressure vessel which ever comes first.

                      Given they were a Acid plant, they should have had a more agressive inspection plan.

                        Geoff you wouldn't happen to have an API 510 certification would you? The above quote sound like the words of an API or an engineer. I have been in inspection since 03 and can tell you many a horror story of lackluster inspection programs and flat out ignoring inspectors recommendations.

                            As soon as you develope a fool proof inspection/maintenance system ; along comes a bigger fool.
                            I was with Amoco R&D 34 yr doing things like metallurgical failure analysis, and writing ASTM, API, NACE specs and standards. I would say the industry/inspector/maintenance does a great job. The general public is amazingly unaware. Refinerys are handling tons/minute of "gasoline" at up to 1100F, etc,etc.
                            Example of what they must deal with: Newcastle DE, maintenance required on a HDPE unit (hexane at 500psig and 500F) . Two guys go to control room to advise and get "hot work" permits. WHILE other 2 guys start dismantling a 6" valve incorrectly - the 4 maintenance guys were in a hurry to get home to watch a baseball game. The valve came apart before the operators had isolated it . Five died and the plant was destroyed.
                            I got a hundred stories.

                              What some issues are is that this plant is small, and its a Petrochemical facility. OSHA is heavily entrained into the Refineries and lots of smaller Petrochemical facilities have little to no resources for Mechanical Integrity programs.

                              This plant in particular is one of these very small sites, that could not provide inspection records because of budget, and basically open admitted to not having a program.

                              Just glad no one was hurt or killed in this one.
